Nun, selbst weiterdenken.

Wo kommen diese Daten her ?

Falls Interesse an Vernetzung bestünde, bitte melden

As the article acknowledges, one cannot infer anything about vaccine effectiveness from this analysis given that it is not age stratified.

As we’ve seen in the other data sets with death outcomes and vaccination status from other countries, it is typical to see death rates considerably lower in every age group in the vaccinated group but the unstratified death rate much higher, a classic example of Simpson’s paradox.

This is caused by the extreme skew in death risk and vaccination propensity by age, with the oldest age groups driving the death data.

The comparisons of % of ICU or dead who were vaccinated vs population vaccination rate is flawed in the same way — comparing the % is not meaningful given the extreme difference in % vaccinated across age groups, lowest in the young groups with very low death risk and highest in seniors with by far the highest.

The article repeated clarifies the author states the data “does not necessarily mean vaccination increased the risk of death or ICU admission” which, while true, greatly overstates the case.

The difference in age distribution assures it is all but certain that is not the case, and a more relevant statement would be the data “does not necessarily mean vaccination did not substantially reduce risk of death or ICU admission”.

Once age adjusted data are available (and preferably also adjusting for other key confounders like preexisting medical conditions) then I expect it to show substantial reduction of risk — very strong in 2021-2022 and decreasing over time (as higher % of unvaccinated have benefit of immune protection from surviving a previous infection) — as the dozens of other studies looking at vaccine effectiveness vs severe and fatal disease from all over the world have shown.

This brings up another point — these data should not be judged in isolation but as part of a large existing literature on potential vaccine effects on severe or fatal COVID.

Avatar von cm27874

I have yet to see a pro-vaccine study for which careful analysis can not identify serious methodological flaws (cases in point: recent French study showing 25% benefit against all-cause death for the vaccinated; RKI's own study using odds ratios instead of relative risk, and building the study in a way that would benefit from a dangerous vaccine, i.e., a vaccine which causes non-Covid hospitalizations). I would LOVE to get my hands on age-stratified data from Germany. Yes, such data might show limited effectiveness for a limited period of time after vaccination, for limited age groups, and against specific variants. Unfortunately, in the case of Germany, such population-level data might not even exist (there is still some hope regarding data of the health insurance companies).

By the way, the more public opinion acknowledges that there is a massive age skew regarding Covid deaths, the more it should become questionable why they locked us down, and forced us into taking new medical technologies.

There exists no study without methodological weaknesses, which is why transparent description of all assumptions, careful discussion of both strengths and limitations, as well as sensitivity analyses assessing robustness to key assumptions is important for all studies, especially observational studies that have numerous inherent biases to deal with.

In some cases, however, like this one, the biases are well-known and fundamental, and failure to address these factors makes the analysis neigh uninformative. Age confounding in this case is one example, for which as has been demonstrated in data sets from other places, death rates can all go in one direction (vax < unvax) for ALL age groups, yet when aggregated and not stratified by age the marginal death rates go the other direction (vax>unvax). This is classic Simpson's paradox, and to ignore this factor is not just a subtle methodological flaw but a fundamental one.

Another age-stratified data set from Germany would certainly be useful, but we already have such data from numerous other places and in numerous other studies, and these should not be ignored if one is interested in answering this question of vaccine effects vs. severe and fatal covid. There is an extensive literature.

And assessing public health policies certainly requires a similarly nuanced perspective, and age stratification is among the factors.

Many make the same error of failure to age stratify in suggesting covid was only a concern for the oldest and most frail, implying nothing should have been done (mitigation or vaccination) for the younger age groups.

While true that marginal risk of death is FAR greater in the older age groups, when stratifying by age the risk of COVID death was non-negligible for all adult age groups, adding significant excess death risk to their age specific background death risk, so intervention was justified, even in retrospect. Of course teh specifics are debatable -- whether the choices made optimized the benefit-risk for each subgroup, and there is the interesting potential debate, especially if we talk about young men, and if we think about economic repercussions.

Avatar von cm27874

I don't know how often I warned against Simpson effects in my posts. I always stressed the importance of age stratification (starting in my very first Substack post, back in 2021: https://cm27874.substack.com/p/credit-for-covid). Yes, Covid had effects on the distribution of deaths in Germany (https://cm27874.substack.com/p/deaths-in-germany-20222023-update, https://cm27874.substack.com/p/deaths-in-germany-20242025-update) but there have been other, larger, unexplained effects. In retrospect (and I am perfectly willing to grant a certain period of panic in the spring of 2020) almost no measures were justified, and the official rhetoric about "looking ahead, going forward, nuanced perspective, risk-ratio optimization" will do nothing to hedge us against totalitarian excesses next time. I am also careful when comparing countries. I suspect that the influence of the general health of the population, of the health insurance system, and the medical system are enormous.

Sure. excess deaths are interesting and important to study and understand.

But with their increased popularity, especially in online discourse, many don't appreciate the fact that excess deaths are not raw data, but inferred quantities from an assumed background death model with a presumed extrapolation from the past providing "expected" over which any observed death count is considered "excess".

Thus, it is not clear from simple "excess" to what degree an increase is increasing death rates, or simply not decreasing at the expected rates, for example. As a result, it is critical to plot both raw death rates (preferably age adjusted o account for shifting population demographics) and. the presumed baseline death rates clearly demonstrated.

Additionally, one needs to keep in mind that there are may potential factors that a could partially explain any increased excess, especially during the pandemic. From data all over the world, it is clear that the vast majority of the excess are covid deaths during covid waves although there are other contributing factors as well.

It is fair to consider potential for vaccine related deaths to be a possible contributing factor, but of course it is not valid to assume all unexplained deaths starting 2021 are caused by vaccines, either.

And I agree the factors you mentioned and many others are all important contributors to the mortality rate over time in any country, and emphasizes why nuanced analysis and interpretation is critically important.

People will always oversimplify the data to push one narrative or another based on their intentions, but it is important to think more carefully, considering all potential key factors, which requires a nuanced approach to avoid buying into false narratives.

Avatar von cm27874

Yes, yes, yes, no, yes, yes, yes to your paragraphs. :)

In an ideal world, you call for nuance would of course be welcome, appreciated and followed. In the world we live in, however, and in the Germany I live in, in particular, those calling for nuance now are yesterday's hardliners who recognize that they are losing.

Thus, why I think it is important to isolate the scientific substance from the political discussions or discussions about political leaders that obviously have their own agendas.
